Form D-4 – Complaint for Violation of the Campaign Disclosure Act

Any campaign disclosure complaint dealing with a candidate or question of public policy appearing on the November 3 General Election Ballot and received by the Board by Monday, October 19 will be brought to the Board for consideration at a special Board meeting to be held prior to the election (date to be determined). The Board cannot guarantee that any campaign disclosure complaint received by the Board after Monday, October 19 will be brought to the Board for consideration before the November 3 General Election and may be considered at the November 17 Board meeting.

In addition, any such complaint filed according to the above-listed criteria must be personally served (not just mailed) upon all respondents prior to the time the complaint is filed with the Board, and the proof of service on the back of the D-4 Form must indicate that it has been so served. Complaints not having been personally served or lacking the requisite proof of service will not be guaranteed a hearing as per the above-listed schedules and may not be heard by the Board until the November 17 meeting at the earliest.
